Genetically, they’d test as siblings.

Yep, if two identical twins have kids with another set of identical twins, then genetically, those children would test as siblings even though they’re cousins.

If one or both sets of twins are fraternal, then nope, because fraternal twins come from two different eggs being fertilized from two different sperm versus one sperm fertilizing one egg that splits off into two embryos later.
